This is the report detailing the findings from the Hull LINk investigation into hospital discharge. On page 21 you will find our recommendations that have now formally been submitted to NHS Hull, Hull & East Yorkshire Hospitals Trust and Hull City Council. Local authorities have to respond to these recommendations within 20 working days.
