Hull and East Yorkshire Dementia, Palliative and End of Life Care Working Group Report
This report describes the local multi-agency development of the dementia long-term conditions integrated care pathway.
Plans to embark upon the pathway development were approved by the local Dementia Strategy Implementation Groups back in March 2010. The report therefore details the process undertaken, the outcomes of this project to date and the next steps.
The report is currently being shared with health and social care commissioners across Hull and East Riding and is being fed into the North Bank Strategic Review for Dementia as well as the Transformation agenda led by Humber NHS FT. The report is also due to be presented to the Hull Dementia Strategy Group on 14th December.
