Hospital Discharge and Transfer of Care Policy

Issue:  Inadequate support on discharge. Policies not being followed. Information given to patients is out of date.

This group has reviewed the actual practice of leaving hospital after treatment including getting transport and the time it takes to be discharged. 

The group has also compared what does happen with what should happen and has looked at whether policy reflects practice. A report with recommendations for improving patient and public experience has now been formally submitted to local authorities. To view the report please click here.
